Santiago arrives in the Central as the new director of Urgencias. His ex-wife Elisa is the boss of nurses at Central and doesn't want to work with him again, so she wants to leave the Hospital and work somewhere else. This is also new nurse Leonor's first day of work. Mario and Diana (SAMUR) go to the site of a school-bus accident. Cecilia is worried because a patient needs a kidney and can't survive for much longer without it; and Susana refutes the invitation of Mario for a date.

Susana continues to refuse Mario's invitations. Rusti tells Mario that Susan is known around the hospital as "La Rarita." Alejandro's son makes his First Communion but is allergic to gluten and becomes ill. Esther and Leonor begin to share a flat. Elisa wants to leave the hospital, but Santiago tells the hospital director not to concede her petition.

Andrea tells Santiago that Cecilia won't allow her to assume responsibilities, so he gives her the case of a woman with a stomach-ache. Unaware that her patient is pregnant, the resident makes a mistake when she asks Rusti to prepare the patient for a radiography. Susana's date with Mario is pre-empted by her mother, who seems to invent illnesses in order to reclaim her attention; Mario tries to meet with her again. Esther and Leonor have their first problems as flatmates, while the SAMUR ambulance is stolen with Diana, Diego, and Mario inside. Javier has a patient ...

A woman comes to the hospital with light contusions accompanied by her husband. It is not the first time she has been there, so Esther suggests that her husband is abusing her. In the hospital there are different opinions about the convenience of talking to the woman about the topic. Esther decides to call her, which unleashes a violent discussion between the couple that culminates in her receiving a brutal beating. A man waits patiently to speak to Susana and ask for explanations about his wife's death, but he loses control and is ejected from the hospital; he promptly returns and throws himself against a window. While Susana attends to him, she gives him the explanations for which he asks. Fashionable singer Richie is brought to Urgencias with a cocaine overdose after his manager Rafa broke the news of his father's death. When Richie recovers, he tells Rafa that they should suspend his tour, but Rafa refuses.

Susana receives an offer to work in a free clinic in Seville and decides to ask for Mario's opinion, but he turns a cold shoulder. She feels responsible for his attitude, creating more tension between them. Finally, she decides to remain because the same clinic will soon open a delegation in Madrid. Meanwhile, a woman brings her adopted son to the hospital, and after many tests, the doctors discover that the boy is suffering from a strange, rare ailment--from which his adoptive father also suffers. Vilches attends to a man with cuts of crystal in his hand, but he prefers to not tell the police anything. When Vilches goes out to the parking lot, he discovers his patient breaking out car windows. He shouts at the man, who rushes at him with such bad fortune that Vilches separates, and the man falls to the soil from a strong blow that ends up costing him his life, and Vilches is considered a murder suspect. They offer retired boxer Victor to spar in exchange for money, but when the moment comes he discovers that his opponent would be his son Toni.

A girl leaves her house at night and is almost knocked down by a taxi. The taxi driver takes her to the hospital and they call her parents, but when they tell her that her parents are coming, she becomes very nervous because she doesn't want to see her father. They suspect that her father has been sexually abusing her and she eventually affirms this theory.

Susana's mother feels sick and goes to the hospital, where tests detect an advanced tumor. Cecilia watches the man who tortured her in Argentina when she was pregnant and loses the daughter she was expecting. The SAMUR team has an intervention in a sewer, but Diana is claustrophobic and Esther must go with Mario. Elisa knows Eduardo, a journalist who is very interested in her. Leonor wants to spend more time with Javier and, as Esther has a guard at night, invites him to her house. Rusti met a woman over the weekend and told her he was a doctor, so when she appears in...

The SAMUR team must give a workshop at a high school; once there they must attend a suicide attempt. Susana returns to the hospital and seems to be a new and happier woman; Mario tries to come back with her, but she advises him to go out with Diana because has noticed that he and the nurse feel something for each other. Eduardo, the photographer who is interested in Elisa is at the hospital researching an article, so Santiago is very nervous. Cecilia tries to find out if her child is still alive. It's Vilches' birthday and Andrea proposes a meeting; he thinks that ...

Santiago faces a disciplinary process after breaking the camera of the journalist who was researching an article at the hospital; though moved by jealousies about Elisa, he believes that he acted in consequences. Vilches's name sounds as substitute, but this one demonstrates loyalty to his friend. Cecilia considers adopting an orphan who is in the hospital with asthma attacks. Esther is chosen as Diana's substitute and works with Mario on the SAMUR team.

Cecilia prepares the "exam" in order to adopt Yolanda and Elisa tries to calm her; however, when the social assistant arrives in the hospital, the nurse tells her that Cecilia lost a daughter, a detail that the doctor hadn't told and that could be harmful for the process. Esther continues replacing Diana as the SAMUR nurse and discovers that she likes very much this new work. Choni tells Leonor that the doctors write each 6 months an inform about nurses an assistants, since this, she is a bit worried about that and asks Javier for telling what he has written about her...

Elisa looks forward to going to Africa with Eduardo, but Santiago isn't happy about it. A fire breaks out at a discotheque and a friend of Alberto's dies. He reunites with his parents but blames his father for his friend's death. Santiago enters a deep crisis and asks Elisa not to go away to Africa and to spend the summer with him. Mario tries several approximations towards Diana, who proves to be unwilling to support a loving relationship. Susana tells Mario who is pregnant.
